The RAFT Dementia Support Program is a FREE state funded program to support older adults living with dementia and their caregivers. Dementia Specialists meet with individuals and their family/caregivers in their homes and provides education, training and support. Families and caregivers learn essential skills to successfully manage the illness and accompanying challenging behaviors, are connected to community resources, and develop an action plan to prevent crises. The goal is to decrease the distress experienced by the individual living with dementia, as well as their family and caregivers.

Making a Referral is Easy!
Anyone can make a referral to the RAFT Dementia Support Program. Referrals can come from individual family members, government agencies, home health care agencies, senior centers, adult day care, hospitals and any other organizations working with older adults.

Dementia vs. Delirium

DEMENTIA is a gradual neurodegenerative process, and is generally not reversible.
It mainly affects memory.

DELIRIUM comes on swiftly, possibly over one to two days, and symptoms can also vary considerably throughout the day. It mainly affects attention.
Can be caused by acute illness, and is reversible.
